¨ Download and fill out your rabies vaccination certificate located above

¨ You MUST bring your completed vaccination certificate with you to your appointment

¨ Wear a Mask and maintain social distant at all times

¨ Do NOT enter the building until your time slot is posted on the door outside the building

¨ 1 person per household is allowed in the building

¨ Dogs must be leashed and cats MUST be in a pet carrier

¨ Copy of Say/Neuter certificate (if licensing for 1st time)
